I am not sure why the median SNAP is normal in TOS, but the medial CMAP is abnormal.

The median sensory fibers are actually in the upper and middle trunks (not the lower trunk, which is involved in TOS), which is why the median SNAP is spared. The median motor fibers (along with the ulnar motor and sensory fibers) are in the lower trunk so you do see abnormalities there.
